Adolescent Wellness Center

The Adolescent Wellness Center (AWC) is a primary care office providing health care services to adolescents ages 10 through 18. AWC provides physicals for work, sports, or college, sick care, gynecological care, pregnancy prevention and STD testing and treatment. AWC also has a social worker available 4 days a week. South Community, Inc. provides a therapist one day a week.

Availability

  • Patients can schedule appointments or walk-in for medical services: call for availability at (937) 222-9355 (WELL).
  • New patients are being accepted, ages 10-17.
  • Most insurance is accepted including Medicaid, Amerigroup, CareSource, and Molina.
  • A reduced fee is available for uninsured families based on household income.

Prenatal Health Services promotes regular prenatal visits beginning in the first three months of pregnancy.
Women’s Health Services (937) 225-4484
The Women’s Health Services Program offers health promotion and disease prevention. A staff of physicians, nurses and a nutritionist provide quality care to women who are uninsured or under-insured.
