Please call (937) 225-4362 prior to making a payment if you have not sent in the required paperwork, or if you do not have the capability to forward it electronically.

For Food Service/Establishment, Temporary Food, Pools, Hotels and Tattoo/Body Art Licensing, you DO NOT have to forward the application unless there are changes (New Owner or Mailing address) or updates (Phone Number or Email).
