Wednesdays (SPANISH ONLY) 12:00 pm - 1:00 pm
Thursdays from 9:00 am - 10:00 am
The Diabetes Self – Management Education Program provides persons 18 or older, who are living with Type 2 diabetes with the tools they need to control their diabetes. This program is certified by the American Diabetes Association and facilitated by a Registered Dietitian.
Program Purpose: To provide all participants with educational endeavors which will influence them to monitor their disease, stressing how to avoid and reduce the rate of complications such as, eye disease, heart disease, stroke, kidney disease, and premature death

Diabetes is the 7th Leading Cause of Death in Montgomery County
Diabetes is a disease that can lead to high cholesterol, high blood pressure, and serious health problems.
- The chance of acquiring diabetes increases with age.
- The odds of becoming diabetic for Blacks are much greater than for Whites.
Our Diabetes Support Group meets the 2nd Tuesday of the month from 11:30 am - 12:30 pm and the 2nd Thursday of the month from 5:30 pm - 6:30 pm SPANISH ONLY- Wednesdays 12:00 pm - 1:00 pm
Class Details: 8-weeks; one-hours sessions held virtually via ZOOM on Tuesdays, Wednesdays(SPANISH ONLY), & Thursdays.
Upon completion of this course you will receive a certificate of completion and an opportunity to schedule a personal consultation with a registered dietician.
