The Public Health – Dayton & Montgomery County Immunization Clinic will be offer walk-in hours for Back to School immunizations on the following upcoming dates:

• Wednesday, August 3 - 12:30 – 4p
• Wednesday, August 10 - 12:30 – 4p
• Wednesday, August 17 - 12:30 – 4p
• Wednesday, August 24 - 12:30 – 4p
• Wednesday, August 31 - 12:30 – 4p

Walk-ins will be taken until the clinic capacity is reached. This is to ensure each of our clients receives adequate attention for their appointment time.

NOTE: A parent or legal guardian must accompany children to their appointment. You must bring the following items with you to your appointment:

• Immunization (shot) record
• Insurance card(s)
• Form of payment (if applicable): cash, check or credit card
• If the person bringing in the child is not the legal guardian/parent, download the Designation of Another Person to Consent for Treatment Form
• Any custody or guardianship papers (if applicable)
• Photo ID for parent/legal guardian
• Wearing a face mask is required
