Vital Statistics – Birth & Death Records
The Office of Vital Statistics provides access to birth records of individuals born in Ohio and death records for individuals who have died in Montgomery County, Ohio.
Individuals may purchase these records online in-person or through the mail. The cost of both birth and death records are $22 each. In addition, if purchased online there is an additional service fee of $7 and a shipping fee if applicable.

Birth Certificate (Order Online)
A certified copy of a birth certificate that can typically be used for travel, passport, proof of citizenship, social security, driver's license, school registration, personal identification and other legal purposes.
Birth Certificates are available for births that occurred in Montgomery County, except Oakwood, from 1909 to present.

- Information related to births prior to 1909 is not available within the State of Ohio records system.
- For births outside the State of Ohio, please contact the state in which the event occurred. Find locations outside Ohio.
Additional Notes for New Birth Certificates
- Certificates for new births typically takes 7 – 10 business days to become available. While Public Health- Dayton & Montgomery County’s Office of Vital Statistics strives to provide this service as quickly as possible, the information must first be received from the birthing hospital (or center) and uploaded into State of Ohio records system.
- The Office of Vital Statistics program processes orders within 7 – 10 business days generally once the information is available.
- If a certificate is not available at the time of your application, the certificate will be mailed via U.S. Postal Service upon coming available.
Death Certificate (Order Online)
Death Certificates are available for deaths that occurred in Montgomery County, except Oakwood, from 1909 to present. (Please note, this is the location of the death and not the previous address of the individual.)
- Public Health can only issues certificates for deaths that occurred in Montgomery County, Ohio from 1909 – present.
- For deaths occurring outside Montgomery County, but within Ohio, contact the County’s Office of Vital Statistics where the death occurred or the Ohio Department of Health.
- For deaths outside the State of Ohio, please contact the state in which the event occurred.
- Information related to deaths prior to 1909 is not available within the State of Ohio records system. For deaths in Montgomery County from 1867-1908 contact the Montgomery County Probate Court. For deaths in Dayton from 1867-1908 contact the City of Dayton.
How to Order by Mail
To order by mail, you must complete pdf Application For Certified Copies (303 KB) and mail it to;

You must also include a payment in the form of certified check or money order. Do not send cash and personal checks cannot be accepted.
Once our office receives and processes your request it will mail your certificate to you.
The office of Vital Statistics is not responsible for mail that is delayed or lost. If you believe mail to be lost or delayed, please contact USPS for assistance with locating your mail.
How to Pick-up A Birth or Death Certificate in Person.
Public Health’s Office of Vital Statistics is located in the Reibold Building in downtown Dayton at 117 S. Main St. A paid parking garage is located with an entrance off of 5th St.
Forms of payment that are accepted are; Cash, Certified Check, Money Order or Credit Card.
If using a money order or certified check, please make payable to Public Health – Dayton & Montgomery County. (No personal checks can be accepted)
